The RN will be part of the Admission Avoidance team, responsible for promoting optimal patient outcomes and ensuring family involvement. You will provide comprehensive, evidence-based care while working with an interdisciplinary team. This role also requires a strong clinical assessment skill set for effective patient management.
Key Responsibilities:
• Deliver direct patient care across various scenarios
• Communicate updates effectively with the healthcare team
• Maintain infection control protocols during care
• Collaborate with multidisciplinary teams on best practices
• Support timely community and program referrals
Requirements:
• Current registration with the College of Nurses of Ontario
• Strong clinical assessment skills required
• Certification in BCLS/HCP essential
• Experience with geriatric patients preferred
• Proficiency in computer literacy necessary
