Position: Community-Focused Practical Nurse Role
Become a vital part of the Hamilton Family Health Team as a Community-Focused Registered Practical Nurse. This full-time position emphasizes mobile healthcare delivery and patient-centered engagement.

In this role, you will travel across HFHT clinic sites, actively supporting patient attachment and continuity of care. Engage unattached patients with commitment, conduct essential clinical onboarding, and work collaboratively with healthcare providers. Candidates must have robust clinical assessment skills and a background in outreach or community health initiatives.

Key Responsibilities:

• Engage with unattached patients proactively

• Complete patient onboarding and chart preparations

• Facilitate warm exchanges between patients and care teams

• Ensure meticulous documentation within EMR

• Collaborate with community partners to strengthen health access

Requirements:

• Registered Practical Nurse in good standing with the CNO

• Two years of experience in clinical or community nursing

• Experience in electronic medical records beneficial

• Strong clinical independence and assessment abilities

• Background in working with diverse and vulnerable populations
