You will join a dedicated team providing evidence-informed care that addresses complex needs in patients undergoing hemodialysis. This position requires flexibility, as shifts may rotate through Brantford, Hamilton, and Ohsweken locations. Bring your experience in chronic disease management as you assume leadership in collaborating with a multidisciplinary team.
Key Responsibilities:
• Provide patient-centred care in hemodialysis units
• Collaborate effectively with interdisciplinary teams
• Assess health needs and treatment response
• Utilize evidence-based strategies for optimal outcomes
• Stay adaptable across multiple care environments
Requirements:
• Current RN registration with the College of Nurses of Ontario
• Minimum 2 years of related nursing experience
• BCLS/HCP certification
• Hemodialysis experience preferred
• Strong leadership and critical thinking skills
