Primary Care Nurse Practitioner

As a Nurse Practitioner at De dwa da dehs nye>s Aboriginal Health Centre, you will provide essential primary care in Hamilton while focusing on culturally appropriate health solutions. Engage with patients to improve their access to health care services.

This role involves delivering care within your scope and addressing chronic and acute health issues. With a minimum of five years in health care, you will work alongside an interdisciplinary team to provide outreach services and ensure comprehensive patient support, including home visits when necessary.

Key Responsibilities:

• Provide comprehensive, culturally safe patient care

• Manage preventive health and chronic illness

• Document and manage patient information effectively

• Collaborate across health services for effective care

• Conduct home visits to underserved patients

Requirements:

• Valid Nurse Practitioner registration with CNO

• 5+ years of direct patient care experience

• Understanding of First Nations and Inuit cultures

• Proficient in electronic medical records systems

• Excellent written and verbal communication skills
