Nurse Practitioner for Resident Care

In this full-time position, your responsibilities will include assessing resident health conditions, prescribing necessary treatments, and participating in care conferences. You are encouraged to provide clinical leadership and engage with families, thereby nurturing comprehensive care for residents within the long-term care system.
Key Responsibilities:
• Conduct health assessments and follow up on care plans
• Work closely with the interdisciplinary care team
• Engage with families in planning and decision-making
• Lead clinical initiatives focused on palliative and dementia care
• Identify and analyze trends for team educational needs
Requirements:
• Minimum one year in geriatric or rehabilitation nursing
• Registered Nurse with Extended Class license in Ontario
• Familiarity with long-term care legislation preferred
• Additional training in mental health or behavior support is a plus
• Previous leadership experience is an asset
