Job Description

Position Summary:

The Registered Nurse (RN) is a regulated health care professional who adheres to and models the Standards and Guidelines of the College of Nurses of Ontario; the SJHH mission, vision and values, policies and procedures; and applicable specialty  the Registered Charge Nurse Post Acute Care Navigator role, the RN demonstrates advanced clinical expertise, systems thinking, leadership, and sound clinical judgment to support safe, efficient, and appropriate patient flow across the organization.

The RN collaborates with the health professional team and assumes a leadership role in coordinating and delivering a full range of care and treatment to patients/clients, including those with complex needs, high acuity, and less predictable outcomes.

The RN is responsible for conducting comprehensive clinical screening and assessment to determine patient appropriateness for Post-Acute placement. This role supports hospital-wide capacity strategies through the coordination and management of patient flow, optimization of bed utilization, and active participation in admission, transfer, discharge, and repatriation  collaboration with physicians, therapy teams, discharge planners, unit staff, consulting services, and external partners, the RN facilitates timely and appropriate transitions of care.

The role balances clinical appropriateness with operational priorities, monitors bed pressures, and proactively identifies opportunities to improve patient flow and system efficiency.

The RN maintains accurate and timely documentation related to screening and patient flow activities and contributes to quality improvement initiatives aimed at enhancing patient outcomes and overall organizational effectiveness.

Qualifications:

Current registration with the College of Nurses of Ontario requiredRNAO, CNA or other professional association membership preferred

Gentle Persuasive Approach training preferred

Demonstrated knowledge, sensitivity and expertise in the care of Post Care changing population

Minimum 3 years of recent related to Post
- Acute care experience with demonstrated knowledge, skill and ability in the care of Post Acute Program required.

Conduct clinical screening and assessment of patients to determine appropriateness for placement within Post Acute Care Bedded Programs.

Systems thinking and ability to balance clinical and operational priorities

Critical thinking and sound clinical judgment under pressure

Coordinate and manage patient flow to ensure optimal bed utilization and support hospital-wide capacity strategies

Liaise with health professional teams (physicians, therapies, discharge planners, unit staff, and consulting teams) to facilitate appropriate admissions, transfers, and discharges

Monitor bed pressures and proactively identify opportunities to improve flow Participate in bed allocation decisions and balancing clinical appropriateness within units

Support timely transitions of care, including repatriation and transfers to alternative care settings

Communicate effectively with internal and external key partners to ensure alignment on patient placement decisions

Maintain accurate documentation related to screening decisions and patient flow activities

Contribute to process improvement aimed at enhancing patient flow and system efficiency

Demonstrated knowledge, skills and judgement required to provide a full range of care and treatment in the clinical area of practice to patients/clients including those with complex needs, high acuity and less predictable outcomes required
