Pediatric RN Community Care Specialist

In this position, you will serve as a Pediatric Registered Nurse, engaging with children and their families to provide tailored nursing care. Your responsibilities will include assessing patient needs, monitoring care plans, and building meaningful relationships, all while enjoying a flexible schedule that promotes work-life balance. The role requires you to travel to patient residences for ongoing support.

Key Responsibilities:

• Use assessment tools for individualized patient evaluations

• Execute and monitor tailored care plans

• Develop trusting relationships with patients and families

• Keep detailed records of patient interactions

• Travel to homes for scheduled patient visits

Requirements:

• Active Registered Nurse registration with the CNO

• Driver’s license and reliable vehicle access

• Experience in community health preferred

• Flexibility for on-call responsibilities
