Palliative Care Case Manager RN

Leverage your nursing expertise as a PT RN Case Manager in Palliative Care, Medically Complex, and Behavioural Programs. This leadership role focuses on patient coordination and care continuity.

We are seeking a skilled PT RN Case Manager to support admissions and discharges while managing a caseload of patients. You will coordinate the MDS Assessment Tool, promote collaborative practice, and integrate evidence-based strategies to enhance care quality. This position requires a commitment to program development and mentoring nursing team members.

Key Responsibilities:

• Manage admissions and discharges effectively

• Coordinate the MDS Assessment Tool for patients

• Ensure continuity of care and support with hands-on tasks

• Promote collaboration among healthcare team members

• Develop programs and contribute to CQI projects

Requirements:

• Graduate of an approved School of Nursing

• Registered with the CNO; BScN preferred

• Experience in effective case management

• Strong assessment skills for complex patient needs

• Proven ability to manage resources responsibly
