Rate of Pay: 41.84 – 46.50 plus $ 2 charge nurse premium.

How You Will Make an Impact Under the supervision of the Clinical Nurse Manager, the Charge Nurse provides clinical leadership and direct nursing care within a dynamic Assisted Living environment. The role supports residents, families, PSWs, and the interdisciplinary team while ensuring safe, compassionate, person-centred care and continuous quality improvement. Assumes additional responsibilities, projects or areas of accountability over time.
Provide clinical supervision, guidance, coaching, mentorship, and competency development to PSWs and other team members. This includes participating directly in resident care when required. Develop, implement, and regularly update person-centred care plans based on resident needs and CNO standards
Working to full scope of practice, you will provide holistic nursing assessments, health checks, vital sign assessments, nursing interventions, clinical emergency response, and ongoing monitoring of resident conditions
Assign and direct care activities based on resident needs, staff competencies, and applicable standards
Oversee medication administration and medication safety, including investigating medication errors, incidents, and near misses and implementing corrective actions
Facilitate care planning, family meetings, and case conferences and collaborate with physicians, pharmacies, healthcare providers, social services, and community partners to optimize resident care Support residents and families through health teaching, problem-solving, safety planning, harm reduction, and access to essential resources
Build therapeutic relationships with residents and their families while respecting resident confidentiality, choice, autonomy, dignity, and individual preferences
Identify staff learning and development needs and collaborate with the Clinical Nurse Manager regarding performance concerns and development plans
Promote health education, disease prevention, quality of life, and continuous quality improvement
Accurately document assessments, care plans, interventions, outcomes, incidents, and required statistics while maintaining confidentiality of personal health information
Communicate significant changes in resident condition and coordinate appropriate interventions
Mentor and support students, volunteers, and other members of the care team Foster a culture of accountability, teamwork, learning, and professional practice
Observe all Good Shepherd health and safety requirements and comply with the Occupational Health and Safety Act and its regulations
What You'll Bring to Our Team Graduate of an accredited Bachelor of Science in Nursing (BScN) program
Current registration with the College of Nurses of Ontario (CNO)
Minimum 3 years of clinical experience working with older adults and individuals with diverse and complex health and psychosocial needs
Minimum 3 years of experience in community, homecare, or other healthcare settings
Experience in a clinical leadership role Demonstrated leadership through quality improvement, mentorship, and training initiatives
Strong nursing assessment, care planning, intervention, crisis management, and risk management skills
Strong understanding of the needs of individuals facing personal and structural barriers to accessing and maintaining housing
Excellent understanding of a harm reduction approach to care Strong communication, interpersonal, conflict resolution, time management, and organizational skills
