You will be responsible for liaising with medical professionals while providing vital direction to Personal Support Workers. Your commitment to patient care will help to ensure seamless communication and continuity of care.
Key Responsibilities:
• Collaborate with Registered Nurse to implement policies
• Notify physicians of significant resident changes
• Coordinate care provisions with health care team
• Direct Personal Support Workers for superior care
• Engage in daily shift reports for continuity
Requirements:
• Background in geriatric and rehabilitation nursing
• Knowledge of governing health care legislation
• Certificate of Competence from the College of Nurses
• Strong ability to work in a team environment
• Dedication to enhancing resident well-being
