As a Visiting Nurse for Bayshore Health Care, you'll play a vital role in enhancing the quality of life for clients in the Greater Hamilton and Burlington area. This position combines your nursing knowledge with personalized client care plans, allowing you to educate clients and their families. Expect responsibilities in medication administration, patient assessments, and multi-team collaboration to ensure ongoing patient wellness.
Key Responsibilities:
• Perform assessments and nursing interventions at home
• Administer and manage medications and treatments
• Update care plans based on patient observations
• Educate clients on disease and medication management
• Work with interdisciplinary teams for care continuity
Requirements:
• Must be a graduate of an accredited RPN program
• Active CNO registration in good standing
• 2-5 years of relevant nursing experience
• Reliable vehicle and valid driver's license
• Flexibility to travel in various weather conditions
