Community nursing in Parry Sound calls for patient assessment and care planning. Para Med schedules visits across varied settings, supporting care at home.
As a Registered Nurse, you will care for patients of all ages with differing complexity of needs. The work centers on helping patients remain comfortable will build therapeutic relationships through rapport, trust, and collaboration while monitoring individualized care plans.
Key Responsibilities:- Assess individualized patient needs using appropriate tools
- Execute and continuously monitor patient care plans
- Build rapport and trust through collaborative engagement
- Maintain and update professional patient records
- Provide required after-hours on-call coverage
- Current good-standing Registered Nurse registration with CNO
- Valid driver’s license and access to a vehicle
- Community care experience considered an asset
- Reliability and flexibility for scheduled visits
- Willingness to travel within a designated area
