Offering services across various environments, you will conduct assessments, aid in diversion planning, and interact with external agencies to fulfill client needs. Key Responsibilities:

• Deliver mobile case management to avert homelessness

• Work with shelter staff for client assessments and referrals

• Partner with community organizations to enhance support

• Accompany clients to housing-related appointments

• Follow Occupational Health and Safety guidelines

Requirements:

• Degree in Social Service or a related discipline

• One year of relevant case management experience

• Familiarity with trauma-informed care principles

• Excellent time management and organization skills

• Valid driver’s license and willingness to drive company vehicle
