Community Case Manager Position

This critical role is suited for candidates with a post-secondary degree in Social Service and at least one year of related experience. You will provide mobile intervention and collaborate with shelter teams and agencies to deliver tailored housing solutions. Your work will embrace principles of anti-oppression and trauma-informed care, ensuring clients receive respectful and effective support.

Key Responsibilities:

• Provide mobile case management for at-risk individuals

• Perform assessments and intake planning with shelters

• Connect clients with housing and support services

• Assist clients at community and housing appointments

• Adhere to safety and compliance standards

Requirements:

• Relevant degree in Social Service or similar field

• Minimum one year of experience in a similar role

• Understanding of diversity and anti-oppressive practices

• Strong communication and conflict resolution abilities

• CPR certification and valid G license required
