Case Management Worker at Booth Centre

Join the Booth Centre in Hamilton as a Case Management Worker dedicated to improving lives through effective support and advocacy. This role is permanent and full-time with a focus on homelessness prevention.

In this role, you will report to the Program Manager and play a significant part in assisting individuals at risk of homelessness. Your duties include conducting assessments, implementing case management strategies, and providing referrals to community resources and supports. You will be an integral part of the Harm Reduction Program.

Key Responsibilities:

• Manage and build a personal caseload of clients

• Complete intake assessments for new residents promptly

• Offer problem-solving support and counseling services

• Establish connections with local support resources

• Maintain accurate records of all client interactions

Requirements:

• Post-secondary education in Social Service or equivalent

• Competence in Microsoft Office applications

• Excellent written and verbal communication skills

• Capability to engage effectively with challenging clients

• Complete necessary background screenings
