Residential Care and Homelessness Caseworker

Support the community as a Residential Care Facilities and Homelessness Caseworker with the City of Hamilton. This essential role involves managing crises and providing targeted support to vulnerable individuals.

You will report to the Human Services Supervisors and focus on delivering specialized assistance to clients with diverse needs. As a subject-matter expert, you will guide both clients and stakeholders through the complexities of the Ontario Works program and the emergency homeless system.

Your role emphasizes crisis intervention, case management, and advocacy.

Key Responsibilities:

• Provide immediate crisis intervention and support

• Liaise with community agencies and stakeholders

• Develop personalized support and outcome plans for clients

• Review and manage financial processes for client assistance

• Document and track client cases across systems

Requirements:

• Degree in Health, Human Services or relevant field

• Knowledge of Ontario Works and emergency protocols

• Proven counseling and case management experience

• Effective problem-solving and crisis de-escalation skills

• Valid driver's license and reliable transportation
