Salary Range: $70,000 - $76,000

Position Summary

The Life Redesign Coach provides trauma‑informed, person‑centered, and culturally responsive coaching support to individuals participating in Hamilton Urban Core Community Health Centre's Residential Treatment and Recovery Program as well as the main HART Hub site. The role supports individuals navigating recovery, stabilization, community reintegration, housing transitions, and life redesign following experiences of substance use, mental health challenges, homelessness, housing instability, poverty, and other complex psychosocial needs.

Working within a recovery‑oriented, harm reduction, and health equity framework, the Life Redesign Coach supports participants in identifying strengths, clarifying goals, and building practical skills to support long‑term wellness, independence, and community integration. The role emphasizes empowerment, dignity, and self‑determination, with a focus on supporting individuals to envision and actively design meaningful life pathways aligned with their values and lived experiences.

Key Responsibilities
  • Support participants in identifying personal strengths, values, interests, and long‑term recovery goals.
  • Facilitate individualized life redesign plans that promote wellness, independence, and meaningful community participation.
  • Assist participants in developing realistic life redesign plans and measurable goals related to housing, employment, education, relationships, health, and personal development to support successful community reintegration.
  • Provide strengths‑based coaching that promotes self‑efficacy, resilience, confidence, and personal accountability.
  • Support participants in envisioning and building a life beyond substance use and crisis through intentional planning and skill development.
  • Monitor progress toward goals and adjust plans collaboratively based on participant needs and priorities.
  • Support discharge planning and aftercare coordination for residents transitioning from the Residential Treatment and Recovery Program.
  • Develop individualized transition plans that support successful reintegration into the community.
  • Ensure participants have appropriate supports in place prior to discharge, including housing, income supports, healthcare access, and community connections.
  • Provide follow‑up support during the transition period to promote stability and prevent relapse or housing loss.
  • Facilitate warm handoffs and continuity of care between residential treatment services and community‑based supports.
  • Support participants in securing safe, stable, and appropriate housing options following treatment.
  • Coordinate referrals and placements into Hamilton HART Hub supportive housing units and other supportive housing programs.
  • Work closely with supportive housing providers, housing workers, landlords, property managers, and community agencies to support housing placement and tenancy success.
  • Assist participants in completing housing applications, gathering documentation, and navigating housing systems.
  • Support participants in understanding tenant rights and responsibilities.
  • Monitor housing stability and provide early intervention to address tenancy concerns and barriers to successful housing retention.
  • Advocate for participants experiencing housing‑related challenges or discrimination.
  • Support participants in developing practical life skills necessary for independent living and long‑term recovery.
  • Provide coaching and education related to:
    Financial literacy and budgeting, Time management and organization, Meal planning and nutrition, Personal wellness and self‑care, Household management and tenancy maintenance, Communication and interpersonal skills, Healthy relationships and boundary setting, Employment readiness and workplace skills, and Educational and vocational planning.
  • Provide ongoing coaching and support to individuals at various stages of recovery.
