Part Time Direct Support Facilitator

Position: Part Time Direct Support Facilitator Role
Support individuals in group living settings with Community Living Hamilton as a Part Time Direct Support Facilitator. Provide social, recreational, and learning activities for clients in the community.

This temporary position offers a chance to improve the lives of adults with developmental disabilities and transitional aged youth. Qualified candidates will have a degree or diploma in a social sciences field and two years of relevant experience. In addition to fostering professional rapport, you will develop Individual Support Plans focused on skill development and wellness.

Key Responsibilities:

• Provide support in social and recreational activities

• Develop Person Directed Plans with clients

• Ensure safety, security, and wellness of clients

• Build effective professional rapport with clients

• Enhance the community's reputation while maintaining boundaries

Requirements:

• DSW, SSW, CYW, or related degree preferred

• Two years experience with developmental disabilities

• Valid NVCI and First Aid/CPR certifications

• Driver's license required

• Fluency in a second language is preferred
