Housing Support Case Manager

Position: Housing Support Case Manager Role
Join Good Shepherd as a Housing Support Case Manager dedicated to transforming lives through individualized support services. This position focuses on aiding clients experiencing chronic homelessness with a hands-on, trauma-informed approach.

As part of the Good Shepherd - Reaching Home initiative, you'll work closely with vulnerable populations, providing essential case management and support services.

Your role includes assisting clients in securing stable housing and coordinating with various community agencies to meet their diverse needs, fostering self-determination and dignity.

Key Responsibilities:

• Deliver intensive case management services to clients

• Facilitate housing searches and coordinate support access

• Attend community appointments and provide home visits

• Collaborate within an interdisciplinary team approach

• Adhere to health and safety protocols at all times

Requirements:

• Post-secondary education in Social Services or similar

• 1+ years of relevant experience in the field

• Knowledge of issues affecting women, children, and families

• Strong interpersonal and organizational abilities

• Must hold a valid G license and willing to get CPR certification
