Group Direct Support Facilitator

Position: Group Living Direct Support Facilitator
Support clients in community-based activities as a Direct Support Facilitator with Community Living Hamilton. This vital role involves engaging clients through social, recreational, and learning opportunities.

As a Direct Support Facilitator, you will work in group living settings, assisting adults with developmental disabilities and transitional aged youth. A focus on respect, dignity, and inclusion will help in fostering effective relationships. Your main duties will include developing Individual Support Plans, ensuring the safety and well-being of clients, and working collaboratively with others for successful outcomes.

Key Responsibilities:

• Provide support in social and recreational activities

• Develop Person Directed Plans for skill development

• Ensure safety, security, and wellness of clients

• Maintain effective professional rapport with clients

• Exhibit conduct enhancing the community's reputation

Requirements:

• Degree or diploma in Social Sciences or related field

• Two years experience with adults having disabilities

• Valid NVCI and First Aid/CPR certifications

• Valid driver’s license

• Additional language skills preferred
