CPS Case Manager Salvation Army

Position: CPS Case Manager at The Salvation Army
Support individuals with developmental disabilities as a CPS Case Manager with The Salvation Army.

This role offers flexible hours and involves planning and implementing goal-centered programs.

In your role, you will participate in providing care for adults in the CPS Lawson Ministries program and the community. The Case Manager will develop supportive and nurturing environments, encouraging independence and meeting emotional needs. Regular evening and weekend shifts along with varied hours are part of the job responsibilities.

Key Responsibilities:

• Plan and implement individual, goal-centered programs

• Address emotional and psychological needs of participants

• Maintain physical needs such as food and shelter

• Promote independence and community inclusion

• Document daily activities and maintain reporting

Requirements:

• Two years' experience working with adults with disabilities

• Completion of Developmental Social Worker Diploma required

• Certification in First Aid, CPR, and Crisis Intervention

• Valid Ontario Class “G” or G2 Driver’s license

• Background check clearance for vulnerable sectors
