In this permanent part-time role with Southbridge Care Homes, you will support the psychosocial needs of residents and families while acting as a vital communication bridge amongst stakeholders. You'll be pivotal in supporting residents during critical transitions and fostering their independence.

Key Responsibilities:

• Participate in admission care conferences and planning

• Deliver social work services to meet residents' needs

• Advocate and provide counseling for residents and families

• Connect families with community resources and advocacy

• Lead discussions on ethical concerns within the team

Requirements:

• BSW/MSW or Social Service Worker qualification

• Registered with the Ontario College of Social Workers

• Preferred experience in healthcare or long-term care

• Empathetic communication and organizational skills

• Awareness of seniors' support services and rights
