Position: Social Worker Role in Acute Care
Provide essential support as a Social Worker eph’s Health System, focusing on acute care and patient-centered practices. This full-time role is dedicated to psychosocial assessments and case management.

The Social Worker position offers a unique opportunity to make a difference in patient care by addressing psychosocial challenges and facilitating effective discharge plans. You will utilize your expertise to assess and coordinate services for patients, ensuring their needs are met throughout their healthcare journey. Collaboration with multidisciplinary teams will be vital to achieving optimal patient outcomes.

Key Responsibilities:

• Conduct in-depth psychosocial assessments and treatment planning

• Facilitate discharge planning and family meetings

• Provide crisis intervention and supportive counselling

• Document patient care interventions per best practices

• Engage in community program development and advocacy

Requirements:

• Bachelor’s degree in Social Work, clinical focus required

• Registration with the Ontario College of Social Workers

• A minimum of two years’ experience preferred

• Exceptional organizational and time management skills

• Ability to thrive in a fast-paced healthcare environment
