Registered Social Worker in Bariatrics

As a part of the Surgical Bariatrics Social Work team, you will provide comprehensive psychosocial assessments and develop intervention plans tailored to patient needs. Collaborate closely with healthcare professionals to ensure coordinated care while leading community linkages and program development in high-risk situations. Your expertise will significantly enhance patient recovery and quality of life for individuals facing weight management challenges.

Key Responsibilities:

• Conduct psychosocial assessments and establish intervention plans

• Collaborate with interdisciplinary team members for care integration

• Document patient interventions following OCSWSSW guidelines

• Identify and intervene in high-risk social situations

• Facilitate therapeutic counselling and case management for patients

Requirements:

• Bachelor’s in Social Work required;
Master’s preferred

• 1 year of experience in Diabetes and Bariatric care

• Registration with Ontario College of Social Workers mandatory

• Familiarity with DSM and psychotherapeutic modalities

• Commitment to patient-centered care in diverse populations
