Part-Time Social Work

Position: Part-Time Social Work Role
Make a difference as a part-time Social Worker across Hamilton's healthcare facilities. Engage in comprehensive psychosocial support and care coordination to aid patient recovery and family education.

This role is classified as temporary and requires a Registered Social Worker, working at Charlton Campus and other facilities. Responsibilities include conducting assessment, counseling, and discharge planning, focusing on enhancing the quality of life for patients. Flexibility and ability to adapt are key for success in this environment.

Key Responsibilities:

• Assess patient psychosocial needs thoroughly

• Develop and implement effective discharge plans

• Document all interventions in compliance with standards

• Manage complex family dynamics with therapeutic expertise

• Provide mentorship to new social work professionals

Requirements:

• Bachelor’s degree in Social Work required

• Registration with the Ontario College of Social Workers is a must

• Experience in acute care preferred (2 years)

• Exceptional organizational and communication skills

• Preferred training in Gentle Persuasive Approach
