Senior Client Services Coordinator

Enhance the quality of life for Métis Nation seniors as the Aging at Home Coordinator. This role focuses on comprehensive client support for individuals aged 55 and older, keeping them independent in their homes.

In this position, you will assess client needs, develop wellness plans, and facilitate access to essential services and supports. This role emphasizes strong community relations and a culturally relevant approach to care, ensuring that every senior receives the resources they need.

Key Responsibilities:

• Ensure the Aging at Home program aligns with Métis values

• Conduct assessments and develop tailored wellness plans

• Provide access to in-home support and friendly visits

• Oversee and process services like lawn care and transportation

• Maintain accurate tracking and reporting of client activities

Requirements:

• Diploma in social services or related field

• 1-3 years working with seniors

• Experience in community service settings

• Proficiency in Office programs

• Ability to transport clients with a valid vehicle
