Elevate lives as a Senior Case Manager with Good Shepherd. This full-time role delivers trauma-informed case management to clients navigating homelessness and housing stability.
In this senior-level position, you will lead impactful case management services for individuals and families with complex needs. Your expertise in Housing First and harm reduction principles will enhance client outcomes throughout the housing continuum. You will also mentor team members and coordinate with community partners to ensure comprehensive support for high-acuity clients.
Key Responsibilities:
• Deliver client-centered case management to high-acuity clients
• Support clients in housing stabilization and follow-up
• Conduct safety assessments and develop safety plans
• Coordinate with shelters and community service providers
• Advocate for clients and maintain confidentiality
Requirements:
• Bachelor's degree in Social Work or related field
• 3–5 years of case management experience
• Strong skills in crisis intervention and teamwork
• Expertise in trauma-informed care and psychosocial rehabilitation
• Valid Ontario G-class driver’s license
