Hours: 35hrs per week, permanent
Salary / Hourly Rate: £37,702 - £38,779

As part of the Physical Disability and Sensory Impairment Team, your role will focus on supporting service users to regain independence through Occupational Therapy principles and practice, enabling them to return home as quickly and safely as possible. You will work closely with individuals undertaking home visits, and facilitate home discharges.

You will play a key role in providing reablement support around activities of daily living, including:

  • Assessment, practice, and evaluation of functional abilities
  • Supporting improvements in independence and confidence
  • Identifying and recommending appropriate equipment where required
  • Making onward referrals to statutory or voluntary agencies to meet identified needs

This is a rewarding opportunity to be part of a supportive, multidisciplinary team, making a real difference to people's lives by helping them regain independence and return home following hospital admission.

You should have the ability to work autonomously and to tight timescales and be able demonstrate responsibility for your own learning needs.

You should have an HNC in Occupational Therapy Support or be willing to work towards gaining this qualification.

Supervision and peer support will be provided regularly.
