Job SummaryWe are seeking a dedicated and compassionate Physical Therapist PT to join our team at Crest Haven Nursing and Rehab, a unique provider of hotel-style healthcare in Middle Township, New Jersey. As a Physical Therapist, you will play a vital role in ensuring our residents receive exceptional care and support to achieve their optimal physical function.
Responsibilities- Conduct comprehensive physical assessments to determine residents' physical impairments and functional limitations, focusing on mobility, balance, and gait.
- Develop individualized treatment plans based on assessments, with clear goals for improving residents' mobility and reducing pain.
- Administer physical therapy interventions, including exercises, manual techniques, and modalities, to enhance residents' physical function.
- Educate residents, family members, and care staff on safe mobility techniques and exercises for continued progress.
- Document resident evaluations, treatment plans, daily and progress notes, and HMO/Manage care progress updates in EMRs (Net Health/PCC).
- Collaborate with the rehab team, including Occupational Therapists and Speech-Language Pathologists, to ensure comprehensive care for residents.
