About the Position:

During visits up to one hour, providers review health, medical history, medications, social environment, and risks, gaining insights that improve care and help close care gaps.

Responsibilities:

What to Expect:

• Conduct assessments, medical history, diagnoses, treatment, education, referrals, follow-up, and documentation
• Collaborate with healthcare team members and family
• Coordinate with primary care physicians on patient education and follow-up
• Provide services in home and skilled facility settings

Position Details:

• Type: PRN - Flexible Nurse Practitioner
• Pay: $1800 to $9000/month (based on 20-100 visits, $90 per assessment)
• Location:
  
  Home, SNF, community in Brookville, IN
• Hours:
  
  Days, evenings, nights, weekends, flexible
• Benefits:
  Non-benefited except 401(k) and voluntary benefits; mileage reimbursement included

Qualifications:

• Master’s Degree or equivalent experience and NP licensure
• Current RN and NP licensure with prescriptive authority
• Board certification (AANP, ANCC, or AACCN)
• Current BLS, ACLS or CPR
• Minimum 1 year NP experience
• Strong computer skills and familiarity with medical records software
• Excellent communication skills
• Flexibility and adaptability to change

Travel Requirements:

• Valid driver’s license, auto insurance, ability to travel to patient locations
