Licensed Physical Therapist Assistant (LPTA)We are seeking a Licensed Physical Therapist Assistant (LPTA) to join our Therapy Department. In this role, you’ll work under the supervision of a licensed Physical Therapist to provide high-quality therapy treatments that support rehabilitation goals and help residents restore mobility, strength, and independence. You’ll collaborate with residents, families, and fellow healthcare professionals to ensure individualized, person-centered care.

- You are a graduate of an accredited PTA program.
- You hold a current Ohio license (or are license-eligible) as a Physical Therapist Assistant.
- You bring at least one year of experience in geriatrics, rehabilitation, or long-term care (preferred).
- You are skilled at motivating residents and supporting them in achieving their therapy goals.
- You have strong communication skills, a collaborative mindset, and a passion for person-centered care.
- You are familiar with therapy best practices, resident safety, and electronic health record documentation.
- Applies physical therapy procedures to patients through use of therapeutic exercise, mechanical traction, therapeutic massage, compression, heat, cold, ultraviolet light, water (whirlpool), and electricity.
- Measures and fits walkers, canes, crutches, and wheelchair, and provides instructions on the use and care of these items.
- Provides instruction in transfers, activities of daily living, gait, pulmonary function, and the use and care of orthosis, prosthesis, and supportive devices.
- Performs measurements without interpretation for range of motion, gross muscle strength, length and girth of body parts, and vital signs.
- Modifies treatment program within the limits of the specific plan of care according to the residents’ needs and response. Reports changes to the supervisor in writing.
- Communicates with staff members and other healthcare professionals on resident’s status and care.
- Complies with OSHA standards and ensures the safety of residents in their care at all times.
- Adheres to infection control policies, professional therapeutic services, and Community First Solution’s standards, policies, procedures, and guidelines.
- Prepares, transmits, and maintains timely and accurate records to include the initial evaluation, progress notes, and attendance records.
- Adhere to all policies and procedures of the organization and all federal and state laws regarding the privacy of protected health information.
- Adhere to all policies and procedures of the organization regarding the use of technology and technology-based information.
- Responsible for welcoming and on-boarding new employees.

- All other duties as assigned and/or appropriate to the position.
- Education:
Associate’s degree from an accredited Physical Therapy Assistant Program. - Licenses or other required certifications:
Current Ohio Physical Therapist Assistant License. - Experience:
