The Registered Nurse (RN) will provide care and services for a group of hospice patients, visiting patients where they reside, which could include private homes, assisted living facilities, and hospitals.
Responsibilities include:
- Completing hospice admissions and patient visits as needed.
- Answering calls from patients, families, and healthcare professionals.
- Conducting nursing visits for symptom management, end-of-life care, and admissions.
- Advocating for hospice services and Suncrest Hospice.
- Working the after-hours on-call schedule: seven days/nights on and seven nights/days off.
- Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice with at least one year of experience under license.
- Admissions and on-call experience preferred.
- Strong teamwork skills: collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership.
- Ability to work independently, managing phone, email, and written communication, and understanding hospice rules and regulations.
- Excellent time-management skills to schedule visits, documentation, and respond to urgent needs.
- Willingness to learn an electronic medical records system.
- Reliable transportation, valid driver’s license, and auto insurance for traveling across the service area.
