Family Connects Nurse Home Visitor

Location:

Salary: $28–$36 per hour

Full Time - 260 days

General Description

Provide home visiting services to all families with newborns in the determined catchment area. Support and assess family needs in physical and psychosocial well‑being. Facilitate referrals and connections to community agencies to support families in all areas. Work and collaborate with others to schedule, visit, and support families in the community using the evidence‑based Family Connects home visiting protocol. Incorporate a strengths‑based approach supporting positive parent‑child bonding and nurturing relationships.

Work with post‑partum parents and/or other caregivers in their home environment to strengthen the family's knowledge of post‑partum health for both themselves and their infant and newborn development. Complete health and physical assessments of birthing parent and baby in the home to determine well‑being. Accurately document and track billable time, and engage a caseload of clients to meet home‑visit number requirements.

Collaborate with other providers to facilitate a “seamless” service system. Appropriately connect and assist families with community resource referrals as needed and work to empower families toward self‑sufficiency.

Qualifications
  • Bachelor’s degree in nursing with current RN license, or B.A. degree in another field with current RN license. Associates degree in nursing with state‑level RN will be considered for position with relevant nursing experience
  • Knowledge of post‑partum health for birthing people and care for newborns preferred
  • Experience and humility to work with culturally diverse families
  • Experience in working with or providing services to children and families preferred; familiarity with agencies serving at‑risk populations and demonstrated ability to work cooperatively with local agencies
  • Experience in establishing partnerships with families
  • Knowledge of agencies and community resources
  • Willingness/ability to work as a team member and independently
  • Ability to work a flexible schedule to meet the needs of families (occasional evening and weekend hours may be needed)
  • Excellent organizational skills and ability to multitask
  • Ability to establish trusting relationships
  • Acceptance of individual differences
  • Willing to engage in building reflective capacity
  • Computer expertise appropriate to the position
  • History of good job attendance
  • Good communication skills in verbal and written
  • Valid driver’s license with good driving record and daily access to private insured vehicle during work hours
  • Must pass criminal background checks and be free from communicable disease
  • Experience in maternal/child health, home visiting, and lactation education, desired
  • Proficiency in Spanish‑written and verbal, preferred

Performance of this position will be evaluated in accordance with provisions of the Board’s policy on Evaluation of Support Personnel.
