• Community Response and Reablement Team: Provide information, advice, and reablement services to new community members. Assist individuals in maintaining their independence within their homes.
• Community Independence and Wellbeing Team: Deliver essential social work and occupational therapy services to individuals across diverse care settings. Promote community integration and support for vulnerable adults.
• Hospital Team Support: Work with the hospital team to deliver comprehensive 7-day social work and care services, supporting individuals, carers, and families during the hospital discharge process. Ensure smooth transitions from hospital to home, ensuring that individuals’ social care needs are met.
• Collaboration with Community Teams: Address the needs of individuals with learning disabilities and mental health challenges. Coordinate with healthcare and social services professionals to provide comprehensive support to service users.
• Community Engagement: Develop and implement strategies to build strong, supportive relationships with the local community. Engage with individuals and families to ensure their care needs are met while empowering them to live independently.
