Role Overview
- Salaried GP at Hammersmith & Fulham Centres for Health, part of LCW (London Central & West Unscheduled Care Collaborative)

- Deliver full range of Advanced Primary Medical Services (APMS)
- Diagnose and manage:
- Acute conditions

- Manage undifferentiated presentations with clinical autonomy
- Develop patient-centred care plans

- Accurate clinical record-keeping
