Position: Social Worker – Referral & Assessment Team (Contract)
Location: Hammersmith

Hammersmith and Fulham | Locum | £39.04 per hour | Hybrid Working

Hammersmith and Fulham Council is seeking an experienced Social Worker to join their Referral and Assessment Team. This is a frontline statutory role responding to new referrals, undertaking timely assessments, and ensuring children and families receive appropriate and proportionate interventions in line with statutory duties.

You will work within departmental priorities and professional standards, contributing to high-quality safeguarding practice and effective multi-agency collaboration.

In this role, you will:

Provide a service in line with statutory requirements and departmental priorities

Undertake referral screening and complete timely Child and Family Assessments

Respond to members of the public and partner agencies via telephone, correspondence, and direct contact

Liaise with police, health, education, and other relevant agencies

Develop and implement individual case plans in line with professional standards

Participate in team meetings, supervision, and required training

Contribute to the emergency duty rota as required

Maintain high-quality written records and use case management systems effectively

Make professional decisions within procedural guidelines, consulting management where appropriate

Contribute to service development tasks and continuous improvement

Actively promote and implement the Council’s equal opportunities policy in practice

What You’ll Need:

Social Work England Registration

Social Work Qualification

At least 3 years’ post-qualified experience in children’s statutory services

Experience with in Referral and Assessment / Front Door services

Strong assessment, analytical, and decision-making skills

Confidence in multi-agency working and safeguarding processes
