Provide basic patient care under the supervision of an RN, including vital sign checks, medication administration, exam prep, specimen collection, procedural assistance, patient education, and diligent documentation while maintaining clear communication with the healthcare team.

Essential duties and responsibilities (including but not limited to):

• Participates in planning, coordinating, and documenting total patient care with the multidisciplinary team.
• Accurately reports and records client condition and care, noting signs and symptoms indicating change.
• Monitors skin assessments and documents changes.
• Completes, maintains, and documents accurate and relevant clinical notes.
• Ensures timely and accurate clinical documentation for all nursing services.
• Participates in education, in‑services, and training as needed.
• Facilitates appropriate nursing actions in response to directives and changing environment.
• Ensures all MD orders are followed and documented accordingly during the shift.
• Manages medication administration, monitoring, and proper storage per policies.

• Graduate of an accredited nursing school.
• Must hold an active LVN or RN license in Louisiana.
• Minimum of 1 year experience as an LVN or RN; experience in behavioral health care preferred.
• Certification in Basic Life Support.
• Certification in non‑violent crisis intervention.
