We’re looking for a Community Lead (North Hampshire) to join our Smokefree Hampshire service. As Community Lead (North Hampshire), you'll lead a team of practitioners, helping them deliver evidence-based smoking cessation support that changes lives across the region.

Working closely with our Head of Service, you'll balance day-to-day operational leadership with longer-term service improvement. You'll use insight and performance data to identify opportunities, remove barriers and support your team to achieve consistently high performance.

A typical day as a Community Lead
might include :

1. Lead a Winning Team
  • Lead a high-performing team by creating an environment where people feel valued, supported, challenged and accountable for delivering the very best outcomes.
  • Set clear expectations, helping every practitioner understand what great looks like, how success is measured, and how they contribute to the wider service.
  • Coach and develop your team to build confidence, capability and consistently high performance, recognising success, addressing challenges early and supporting people to achieve their potential.
  • Foster a culture of openness, collaboration and continuous learning, where people feel empowered to share ideas, challenge constructively and improve together.
2. Drive Performance
  • Lead the operational delivery of the service across your locality, ensuring it consistently delivers against contractual objectives, local priorities and key performance indicators.
  • Use data, insight and local intelligence to identify opportunities, remove barriers and improve performance.
  • Maintain a close understanding of team and service performance, taking timely action to improve referrals, waiting times, quit dates and service user outcomes.
  • Work collaboratively with the wider Smokefree Hampshire Leadership Team and Head of Service, taking collective ownership of the service, sharing learning, escalating risks early and contributing to solutions that strengthen performance across Hampshire.
3. Deliver Outstanding Quality
  • Ensure every service user receives safe, evidence-based and person-centred smoking cessation support, delivered in line with national guidance and local service standards.
  • Create a culture where quality, professionalism and continuous improvement are embedded in everyday practice.
  • Maintain high standards of clinical governance, safeguarding, information governance and record keeping, taking prompt action to address concerns and manage risk appropriately.
  • Use audits, service user feedback, incidents and service insight to identify learning, share best practice and improve the quality and consistency of the service.
4. Build Trusted Partnerships
  • Build and maintain strong relationships with GP practices, pharmacies, NHS partners, community organisations and other local stakeholders to increase referrals and improve access to the service.
  • Act as an ambassador for Smokefree Hampshire, ensuring the service remains visible, responsive and trusted within the communities we serve.
  • Work with local partners to improve access for priority populations and reduce health inequalities.
  • Share knowledge and collaborate with colleagues across Thrive Tribe, recognising that the best outcomes are achieved through strong partnerships and collective working.
5. Working as One Team
  • Work collaboratively with the wider Smokefree Hampshire Leadership Team and Head of Service, taking collective ownership of the service and recognising that the best outcomes are achieved when we work as one team.
