Physician — PRN, Hampton, VA

- Perform comprehensive assessments and develop individualized care plans for residents in post‑acute and long‑term care settings
- Diagnose, treat and manage acute and chronic medical conditions consistent with current evidence‑based practice
- Provide medication management, including review of polypharmacy and coordination of medication reconciliation
- Respond promptly to acute changes in condition, initiate timely interventions and coordinate transfers of care when necessary
- Collaborate with nursing staff, advanced practice clinicians, therapists, social workers and facility leadership to deliver holistic, person‑centered care
- Participate in facility rounds, care‑planning meetings and interdisciplinary case conferences
- Document clinical encounters accurately and promptly in the Electronic Medical Record (EMR)
- Support medication safety initiatives, including antimicrobial and antipsychotic stewardship
- Provide clinical consultation and education to facility staff and medical colleagues on complex clinical issues and best practices
- Maintain professional standard of practice, including infection control, documentation and regulatory compliance
- Doctor of Medicine (MD) or Doctor of Osteopathy (DO) from an accredited institution
- Active and unrestricted Virginia medical license
- Board certification or eligibility in an appropriate specialty (family medicine, internal medicine, geriatrics) preferred
- Clinical experience in post‑acute care, long‑term care, geriatrics or related settings preferred
- Proficiency with Electronic Medical Record systems and clinical documentation
- Strong clinical judgement, interpersonal and communication skills; ability to work effectively within multidisciplinary teams
- Commitment to patient‑centered care, cultural sensitivity and ethical practice
- Current Basic Life Support (BLS) certification;
Advanced Cardiac Life Support (ACLS) preferred where applicable
