Center Clinical Director, Associate

Job Overview

The Clinical Director will directly supervise and train primary care providers (PCPs) in his/her assigned center. The incumbent is accountable for maximizing core model execution, improving clinical quality, efficiency, outcomes, and clinician/patient satisfaction. In addition to the overall clinical outcomes of the center, the Clinical Director will spend a portion of time on direct patient care and leadership duties.

• Provide independent care for patients with acute and chronic illnesses among older adults.
• Take full accountability for patient care and outcomes, seeking specialist consultation as needed.
• Engage with the hospitalist when any patient is in the hospital.
• Conduct assessment, diagnosis, treatment, management, education, health promotion, care coordination and documentation for complex chronic needs.
• Lead the care team (care promoter, care facilitator, care coordinator) for patients able to come to the office.
• For patients unable to come to the office, coordinate with transitional care teams, case managers, physicians, and other resources.
• Lead weekly Super Huddle and Transforming Care Meeting, and support center clinical leadership as needed.
• Monitor daily center census, contributing to joint center accountability for outcomes.
• Participate in recruitment and interviewing of patients and additional providers.
• Mentor and coach PCPs to deliver outstanding clinical outcomes.
• Collaborate with Center Operations Director/Market General Manager to improve financial performance and increase center membership.
• Perform other duties as assigned by management.

• Proficient with Microsoft Office Suite (Word, Excel, PowerPoint, Outlook) and company applications.
• Willingness to travel locally, nationally for initial training, and regionally or nationally for recruiting or training.
• Fluent in English; fluency in additional languages may be required by center.
• Capable of independent judgment and decision-making.

• MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar specialty.
• Current, active MD licensure in the state of employment.
• Minimum of 1 year clinical experience in geriatric, adult, or family practice preferred; lead PCP ideally a Chen Med PCP Partner.
• Completion of Chen Med training, including Masterful Conversations and meeting facilitation.
• Board certification in the relevant specialty preferred; board eligibility is required.
• Maintain board certification through MOC, CME, or reexamination.
• Current DEA number for schedule II‑V controlled substances.
• Basic Life Support (BLS) certification from AHA or American Red Cross required within first 90 days.

Pay range: $238,832 – $341,189 (base salary). Compensation may include bonuses or commissions.
