Annual salary: $49,612.
Work schedule:
Monday – Friday, 8:30 am – 5:00 pm.
- Develop personalized care plans and coordinate services for individuals with intellectual and developmental disabilities.
- Advocate for clients, provide counseling, and offer crisis intervention.
- Support daily living activities to promote independence and community inclusion.
- Monitor client progress, update care plans, and facilitate connections with community resources.
- Bachelor’s degree in human services, social work, psychology, or counseling.
- At least one year of experience in support coordination services with adults having developmental disabilities.
- Valid driver’s license.
- Strong communication, empathy, organizational skills, and flexibility.
- Ability to pass a criminal history fingerprint background investigation, DMV record check, Child Registry search, drug screening, and employment reference checks.
