Your role as a Case Manager

As a Full-Time Case Manager specializing in Substance Use Disorder at the Hampton-Newport News Community Services Board, you will play a critical role in ensuring continuity of care for individuals with mental illness, intellectual developmental disabilities, and substance abuse challenges. This dynamic position involves assessing individual needs, developing tailored service plans, and providing hands‑on support in areas such as crisis intervention, basic needs assistance, and medication supervision.

You will engage with individuals and families to deliver compassionate advocacy while coordinating services across various disciplines. The role requires creativity and adaptability to navigate resource limitations, especially in crisis situations.
