Responsibilities
Evaluate, diagnose, and develop individualized treatment plans for residents with speech, language, cognitive-communication, and swallowing disorders.
Deliver evidence-based therapy services in accordance with physician orders and facility policies.
Collaborate closely with an interdisciplinary team, including Physical Therapists, Occupational Therapists, Nurses, and Physicians, to ensure comprehensive care.
Educate patients, families, and caregivers on treatment goals, home programs, and progress.
Maintain accurate and timely clinical documentation in compliance with state, federal, and facility guidelines.
Participate in care plan meetings and contribute to discharge planning as needed.
Master's Degree in Speech-Language Pathology from an accredited program.
Current State Licensure (or eligibility for licensure) as a Speech-Language Pathologist.
Certificate of Clinical Competence (CCC-SLP) from ASHA (CFY candidates welcome to apply).
Prior experience in a Skilled Nursing Facility or similar setting preferred, but not required.
Strong interpersonal, organizational, and communication skills.
