Riverside Mental Health and Recovery Center in Hampton, VA seeks a Part-time Care Manager to coordinate patient care and discharge planning across the continuum. You will act as the initial contact for patients and families, collaborating with the treatment team to ensure appropriate aftercare plans and effective use of resources.
The role requires a Bachelor's in a related field and 1 year of clinical experience; CPR/BLS within 30 days and CCM/ACMA certifications are preferred to enhance
