At Matrix Medical Network, we are looking for passionate Nurse Practitioners or Physician Assistants to conduct comprehensive in-home health assessments for adult and geriatric patients. As a Matrix provider, you will:

• Conduct thorough assessments, including medical history, diagnosis, treatment, and health education.
• Administer and order necessary point-of-care and lab-based testing.
• Collaborate with physicians, case managers, and caregivers to ensure comprehensive care.
• Provide services in the comfort of the member’s home or skilled nursing facilities.

• Flexibility: Schedule visits based on YOUR availability, with assessments conducted from Sunday to Saturday, 8 am – 7 pm.
• Competitive Pay: Earn between $1,800 – $11,500 per month based on the number of assessments completed.

We’re looking for candidates who possess:

• Master’s Degree or equivalent experience with NP or PA licensure.
• Current NP or PA licensure with prescription authority.
• Board certification in relevant specialties.
• Current BLS, ACLS, or CPR certification.
• 1 year of experience preferred, but new grads are encouraged to apply!
• Strong communication skills and proficiency in medical record software.
