Monday – Friday, 9:00am – 5:00pm Flexible Start Time | Remote Charting | No Weekends

We are seeking an experienced and compassionate Nurse Practitioner (NP) OR Physician Assistant (PA) to join our Long-Term Acute Care (LTAC) team as a Hospitalist.

This is a day-shift, Monday through Friday role with consistent hours and no nights.

The NP will collaborate with physicians, nurses, and interdisciplinary team members to manage complex patient populations, ensure continuity of care, and promote positive patient outcomes.

• Provide comprehensive inpatient care for LTAC patients under the supervision of attending physicians
• Perform daily rounds, patient assessments, and care plan updates
• Order, interpret, and evaluate diagnostic tests and treatments
• Manage acute and chronic conditions within scope of practice
• Collaborate with the care team to ensure seamless transitions of care
• Document all patient encounters accurately and timely in the EMR
• Serve as a clinical resource and patient advocate

• Current Nurse Practitioner license (APRN) with prescriptive authority in [State]
• Certification as an Acute Care NP, Adult-Gerontology NP, or Family NP (with inpatient experience)
• Minimum [2+] years of clinical experience, preferably in hospitalist, acute care, or LTAC settings
• Strong clinical judgment and ability to manage medically complex patients
• Excellent communication and collaboration skills
