Multiple Shifts Available: 8 Hour,10 Hour and 12 Hour Shifts Available Paid one on one training and education provided. ​ Empower, engage and train dialysis patients on peritoneal and home hemodialysis using state-of-the-art training programs and equipment.​ Primarily works in the clinic setting and conducts initial home visits with another staff member present​. Position functions as an onsite Patient Educator and Case Manager for patients with various shifts and work hours​.

In this role, you will work out of our Cherry Park/Clackamas Home Therapies office, generally Monday through Friday (flexible to patient needs). Patients will come to you for 1:1 training as they prepare to transition from receiving their dialysis treatments in our outpatient clinic to performing their own dialysis treatments in the comfort of their home, often even as they sleep!

This individual will assess patient and document findings, administer medications, as well as assess and address patient/family educational needs regarding end-stage renal disease, dialysis therapy, diet and medications. PURPOSE AND SCOPE:
Supports FMCNA's mission, vision, core values and customer service philosophy. Adheres to the FMCNA Compliance Program, including following all regulatory and FMS policy requirements. Functions as a member of the Home Dialysis Team as the Home Therapy Staff Nurse. Ensures provision of quality Home Dialysis Training and patient care in accordance with FMS policies, procedures and the Ultra Care at Home principles.
