The QIDP II is central to the overall responsiveness and effectiveness of an individual’s active treatment program at an SSLC through oversight of the Individual Support Plan (ISP) process. The role involves coordination, monitoring, documentation, and follow‑up on all programs, services, and plans for the individuals on the caseload. It includes leading the interdisciplinary team (IDT), ensuring timely training and oversight, and promoting competent interactions of staff with individuals in program implementation and behavior management.
EssentialJob Functions
- Attend work on a regular basis and follow the agency leave policy, including possible on‑call status.
- Orchestrate and oversee the ISP process for all individuals on the assigned caseload, including facilitation of IDT meetings.
- Ensure assessments and evaluations are conducted to determine current functioning, preferences, strengths, and needs.
- Coordinate participation of the individual and, as appropriate, the individual’s Legally Authorized Representative with staff in the development and delivery of needed supports.
- Ensure follow‑up occurs for any recommendations for supports, services, equipment, or programs.
- Observe and interact with the individuals across settings in program areas.
- Collect and analyze data on ISP implementation and effectiveness.
- Coordinate with the IDT regarding modifications due to progress or lack thereof.
- Train and coach staff in the implementation of services and behavior management.
- Provide leadership and ad‑hoc training to staff on specific issues related to assigned individuals.
- Work with behavioral health staff to integrate behavioral needs into the ISP.
- Monitor and evaluate implementation and documentation of ISP elements monthly.
- Document an analysis of information in the monthly review document.
- Ensure client protections and address significant life events promptly.
- Follow the Human Rights Committee process for restrictive practices and act as advocate.
- Maintain electronic records in compliance with ICF/IID standards.
- Participate in disaster response, continuity of operations, or similar duties as assigned.
- Knowledge of the IDT process and people‑centered planning.
- Knowledge of rehabilitation and skills acquisition programs for persons with intellectual disabilities.
- Knowledge of or ability to develop plans such as Positive Behavior Support Plans and Physical Nutritional Management Plans.
- Ability to coordinate programmatic and clinical services and supports.
- Ability to prepare and maintain records.
- Ability to communicate verbally and in writing clearly and constructively.
