What You Will Do

Manage and support a caseload of service users
, particularly those dealing with opiate substitute treatment and complex substance misuse issues.

Conduct assessments, develop recovery plans
, and review progress using a person-centred
, strengths-based approach.

Engage in community outreach
, meeting clients in various settings, offering support, delivering interventions, and encouraging service access.

Provide harm reduction advice
, including needle exchange, BBV awareness, and safer-use education.

Work closely with multi-agency partners such as housing, health, social care, and criminal justice to support clients’ holistic recovery needs.

Maintain accurate and up-to-date case records
, ensuring confidentiality and timely documentation in the service’s case management system.

Monitor and respond to safeguarding concerns, ensuring the safety and wellbeing of clients at all times.
