The role will include tasks such as:
- Helping people to leave hospital as soon as is practicably possible and to safely transition back to living at home as independently as possible.
- Visiting Service Users in their own homes to provide appropriate care and support as defined in the Care Plan.
- All aspects of personal care, administering medication, monitoring of vital signs, exercise and domestic tasks if needed
- Liaising with medical proffessionals such as district nurses and pharmacies.
Working hours are between 7am-10pm with shifts patterns of 7am-2pm and 4pm-10pm which are inclusive of alternate weekends.
We offer high pay rates of £13.43ph on weekdays and £15.50ph on weekends.
- the hourly rate is subject to holding a NVQ level 2 or above in Health and Social Care
- Flexible contracts available of up to 40 hours a week
- Paid mileage at 40p per mile
